One of the major steps in development of a reliability program using Reliability Centered Maintenance (RCM) is a Failure Modes & Effect Analysis (FMEA). An FMEA is a step by step process to identify the potential failures in a system, equipment, process, or procedures. They are effective in a range of industries from refining, mining, commercial facilities, healthcare, and water treatment, to name a few.
An FMEA is composed of two parts, and you guessed it!

- Effects Analysis
Failure modes are how the system, equipment, procedure, or process could fail to meet it’s objective. It’s critical to define the goals of the organization to better understand a failure.
